Looks like one of the tiny Israeli-flag-on-a-toothpick things that get handed ouut, or given away, at some pro-Israeli events. (I ordered some products from an Israeli website a while back; they came in a timely fashion, and two toothpick-flags were enclosed. I've also seen them at fairs and such.)
I suspect that Gov. Palin was handed one on some forgotten occasion, and she chose to keep it. Personally, I don't think this says much about her feelings vis-a-vis Israel, one way or the other.
